The following are health forms you may need for school this year:
Physical Form.
Medication Authorization Form,
Dental Exam Form, or
Dental Exam Waiver Form,
Eye Exam Report,
Eye Exam Waiver,
Allergy Action Plan,
Diabetes Medical Management Plan.
The Illinois School
Code and Rockton School District require that every child present evidence of a
physical examination and immunizations within one year prior to entering
kindergarten, 6th and 9th grade. Rockton School District is requiring all
medical records in full compliance before the first day of school. All
students entering pre-kindergarten through 7th grade are required to receive the
varicella vaccination (chicken pox). They must receive the vaccine or provide a
statement from their doctor or health care provider verifying the child has had
the disease.
Parents who object to immunizations or health examinations for their children on
religious grounds must present a written, signed statement of such objections.
General philosophical or moral reluctance to allow immunizations and health
examinations will not provide a sufficient basis for an exception to statutory
requirements. Objections to immunizations for medical reasons must be documented
with a signed statement from a physician licensed to practice medicine.
A dental exam is required for kindergarten, 2nd and 6th grade. It must be
completed on the required Illinois state form. Either the dental exam form or
dental waiver form needs to be turned into the school by May 15th. This form
does not have to be completed for the first day of school.
All Illinois children in kindergarten or upon first entry into an Illinois
school beyond kindergarten (grades 1-12) are required to have an eye exam. A
licensed optometrist or ophthalmologist must perform the examinations. Each
child is required to present proof of an eye examination on the Eye Examination
Report form or the Eye Examination Waiver form prior to October 15th of the
current school year. School eye examinations must have been completed within the
12 months prior to the October 15th deadline.
Please refer to the student handbook for school policy on both prescription and
over the counter medication at school.
Attendance is very important for your child’s success in school. Refer to the
sick day guidelines above or student/parent handbook for guidelines that may be
helpful to determine if your child is ill enough to stay home from school.
Please remember to call the school if your child is going to be absent and give
the date and reason by 8:00 AM. A message can be left on our voice mail anytime
if your child will not be attending school. Your child will be marked unexcused
until we have contact with a parent or guardian. Unexcused absence could result
in a referral to the Regional Attendance Office.
