Notes from the Nurse:T
The Illinois School Code and Rockton School
District require that every child presents evidence of a physical examination
and immunizations within one year prior to entering kindergarten, 6th and 9th
grade. Rockton School District is requiring all medical records to be
in full compliance before the first day of school. Please be aware for
the 2009-2010 school year, all students entering pre-kindergarten through 7th
grade are required to receive the varicella vaccination (chicken pox).
They must receive the vaccine or provide a statement from their doctor or health
care provider, verifying the child has had the disease. A school physical
will work as a sports physical as long as the doctor checks "yes" for
interscholastic sports for one year.
A dental exam is required for kindergarten, 2nd
and 6th grade. It must be completed on the required Illinois state form.
Either the dental exam form or dental waiver form needs to be turned into the
school by May 15th. This form does not have to be completed for the first
day of school.
Also, school code states that all new students to
the state of Illinois must have an eye examination. We are requiring these
by October 15th, 2009.
Please be aware that physicals may be turned in
before the end of the year to Whitman Post Elementary School.
