Mrs. LaForge Notes from the Nurse:T
Welcome to Whitman Post Elementary School! All out of
state students enrolling in our district for the first time need to show
proof of a current physical and immunizations. The physical form needed is
listed above. Hepatitis B vaccine is a requirement for 5-8th grades.
Physicals and immunizations need to be in by the first day of school and I
will help in any way I can. If you have further questions or special
concerns regarding your student, I will be in my office, off and on, the
week before school starts. Feel free to call me at 815-624-4006. Thank you
